Medical Forms
Also, review, print and and complete the medical release forms for the Springfield Boys' Volleyball School.
Medical Release forms and the remainder of your payment must be sent to Coach Sullivan no later than June 22, 2011.

Parents
Upon receipt of application and deposit, you will receive confirmation including directions, a list of equipment needs, and other helpful information. Campers must provide their own bedding (sheets, pillows, and blanket).
Remember, Check in is June 26, 2011 from 1 - 2:30pm.
